This information is intended to answer questions you might have concerning our practice. If you have questions we have not answered, please ask. We feel that informing you of our policies before we establish a professional relationship will help prevent misunderstanding later.
Office hours are from 9:00 A.M. to 5:00 P.M., Monday through Friday. We try to schedule all appointments to minimize waiting time and to provide optimal care for each patient. If you can’t keep your appointment, please call and cancel so we can offer this time to another patient.
Minors: For your and our protection, we will not treat a minor unless accompanied by a parent or guardian. An exception to this policy is a notorized copy of the Authorization To Consent To Healthcare for Minors. A minor is anyone 17 years old, or younger, with the exception of a married female or male.
Release of information: Reports concerning your health will not be released to an insurance company, third party, health care provider, family and/or friend without your written authorization. Release of information forms are available in the forms section of this website.
Appointments: To make an appointment at our office, please call 253.770.9000. Of necessity the physician must work by appointment; however, due to emergencies and the unexpected, he will occasionally get behind. Infrequently your appointment may have to be rescheduled for another day or time. We request your patience and cooperation if this should occur.
Emergencies: If you have a serious medical problem after hours, call our office at 253.770.9000. Our answering service will contact one of our physicians. One of our physicians is always on call to respond to your ear, nose and throat problems. In case of a life threatening emergency, go immediately to the nearest hospital emergency room.
Prescription refills: Prescriptions or new medicine request will be filled only during office hours. After hours calls that require any refills or new prescriptions will be directed to the emergency room. Post-surgical patients should ensure that they have an adequate supply of medicine for weekday and weekend coverage.
Many of our patients are allergic to tobacco. Since exposure to tobacco smoke is detrimental to their health, you are asked not to smoke in or around our office. Please do not bring food or gum into the office. Also, please turn off your cell phones prior to entering the exam room.
