Ouroffice is located near Stevens Hospital in Edmonds, Washington.Our physicians and staff offer state-of-the-art services for all ankle and foot problems and disorders.We provide excellent care, taking time to educate our patients about their foot and ankle problems, explain appropriate treatment alternatives, and provide the ideal treatment for them. We treat patients of all ages, emphasizing preventative care. We have recently completed expansion of our office space. We have convenient hours that allow for timely appointments, and we always have a doctor on call for emergencies.
Insurance Billing: Each patient needs to understand that they are responsible for all fees. As a courtesy to our patients, our billing department will submit claims to your insurance company. It is the patients responsibility to keep us informed about any insurance changes.
Co-Payments: Patients whose insurance requires a co-payment are expected to pay the specified amount at the time of each appointment, or a $10.00 administrative fee may apply.
Referrals: If your insurance requires a referral or authorization, please contact your primary care provider to be sure this is received prior to your scheduled visit.
Scheduled Appointments: Your time is important to us and we make every effort to stay on schedule. Please understand that on occasion the doctors need to see emergency patients, which may cause them to be behind schedule. Our staff will keep you informed of any delays.
Late Arrivals: In consideration of other scheduled patients, if you arrive more than 15 minutes late for your appointment time, we may need to reschedule your appointment.
Appointment/Surgery Cancellations: There is a charge of $25.00 for appointments that are missed or cancelled with less than 24 hours notice. Surgery cancellations are subject to a $100.00 fee if an advance notice of at least 24 hours is not given.
Privacy Notice: New laws and HIPAA regulations require that we keep your appointment private and your medical information confidential. Private health information is used as a basis for planning your care and treatment. It is also a means of communication among the other health professionals who contribute to your care. A copy of the Notice of Privacy Practices will be given to you upon request.
